Ordering a Choose Life license plate is easy:

1.  Download and print the application:  CLM_application_form.pdf

2.  Send the application, along with a check or money order for $40.00 to the address on the form. 

     Please make checks payable to:  Choose Life of Maryland, Inc.

3.  CLM will add you to their membership roster and send a MVA form back to you.

4.  Complete the MVA form and return it to CLM (same address).

5.  You will receive your new plates, registration, and decals in 3-6 weeks.
